Recent Inspection Report Shows Grove of Evanston Still Underperforming
Last year, The Illinois Department of Public Health (IDPH) slapped the Grove of Evanston, 500 Asbury St., with a $25,000 fine for a Type A violation where “there is a substantial probability that death or serious mental or physical harm will result, or has resulted.” According to news sources and IDPH officials, the nursing home violated the terms of its license when it failed to assess a resident and notify a doctor of his declining condition. IDPH said the failure resulted in the resident being sent to a local hospital where he died less than 24 hours later.
Public health officials reviewed the resident’s records and recently released the report findings to show:
- The assistant director of nursing noted there had been a decline in the man’s condition for three consecutive days.
- A nursing director admitted to investigators she did not notify the resident’s doctor despite the man’s functional decline.
- The doctor said he only learned of the resident’s change in health status after the sickened man was found sitting on the floor in shock, and lethargic with abnormal vital signs.
The Grove of Evanston is a 124-bed facility licensed through the entity Evanston Skilled Nursing Facility LLC. State and federal records show Evanston Skilled Nursing Facility LLC is a for-profit organization with an address of 801 Adlai Stevenson Drive in Springfield.
A History of Understaffing and Neglectful Care Practices
In the last three years, The Grove has been issued nine citations and one federal fine, according to U.S. Department of Health and Human Services inspection reports. Care and neglect issues are mostly related to understaffing, which is consistently rated “below average” by the U.S. Centers for Medicare & Medicaid Services. In the most recent federal health inspection included in the Third Quarter Report of Illinois Nursing Home Violators for 2019, seven health citations were issued to the Grove. They included failing to protect a resident from physical abuse, failure to maintain hearing aids, failure to dispose of expired medications and failure to clean its plastic trays.
