List of 22 Seriously Under-Performing Nursing Homes in Illinois Released Publicly for First Time 

After an inquiry led by U.S. Senators Bob Casey (D-PA) and Pat Toomey (R-PA), the Special Focus Facility (SFF) program, overseen by The Centers for Medicare and Medicaid Services (CMS), shared a list of nearly 400 consistently underperforming nursing homes, 22 of which are located throughout Illinois. Previously CMS did not publicly disclose the names and locations of these SFF identified facilities. These are nursing homes that if not improved or fail to provide resolutions to documented quality problems, can be cut off by Medicare and Medicaid funding and support.

On June 3, 2019, the concerned lawmakers published the list in a public report titled, Families’ and Residents’ Rights to Know: Uncovering Poor Care in America’s Nursing Homes, and included all of the homes CMS has deemed to have a “persistent record of poor care” and systemic shortcomings.

Unfortunately, this list was not made public prior and only shows a fraction of those who should be included. Federal budget cuts in 2014 reduced the number of available slots for homes to be identified in the SFF program and only a minority of facilities were selected for participation as a result of limited resources and funding at CMS. A more comprehensive list would perhaps use CMS starred ratings on the Nursing Home Compare website, including 2,900 nursing homes with the lowest one-star overall rating, 400 of which the Senators’ report shows the greatest likelihood of putting residents at risk of abuse or neglect.

New List of Illinois’ Worst Nursing Homes  

The Associated Press reports that about 1.3 million Americans are nursing home residents, cared for in more than 15,700 facilities, about 3 percent of which are part of the SFF program. In Illinois, 22 SFF homes made this most recent list and included:

  11. Franklin Grove Living and Rehabilitation
  12. Gardenview Manor
  13. Generations at McKinley Court
  14. Generations at McKinley Place
  15. Greentree of Bradley Rehabilitation
  16. Helia Healthcare of Belleville
  17. Helia Healthcare of Champaign
  18. Landmark of Richton Park
  19. South Elgin Rehab & HCC
  20. Suburban Nursing & Rehab Center
  21. Swansea Rehab Health Care
  22. Willow Crest Nursing Pavilion

Many of these homes also made a list Levin & Perconti nursing home abuse and neglect attorneys recently reviewed from the Illinois Department of Public Health’s (IDPH) first Quarterly Report of Nursing Home Violators for 2019. These facilities were cited for a range of abuse, safety and neglect violations of the Nursing Home Care Act but ironically, many were not included in the greater SFF list.
